All renters and guests of owners must use this form to register their stay.
- Pets are not allowed – violators will be fined $100 per day.
Community Registration
Under Federal ADA regulations (28 C.F.R.§35.104), a service animal is a dog that is individually trained to do work or perform tasks for the benefit of an individual with a disability. Because service animals are task-trained, medical documentation is not required, and this form is limited to the questions permitted by law.
This form is for Service Animals as defined under the ADA. Medical documentation is not required.
Use this form if you are requesting an Emotional Support Animal (ESA) as a housing accommodation. Under Florida Statute §760.27, an ESA provides therapeutic emotional support by virtue of its presence and does not require task training. ESA requests require supporting documentation from a licensed health care professional. Florida Statute §817.265 addresses misrepresentation of ESA documentation.
Fines (per adopted schedule): Failure to pick up after a pet: $50 per incident. Other rule violations: $50 (first offense) per day; $100 (second offense) per day.

Room is available between 8:00am and 11:00pm for owners and renters upon request and approval.
Kitchen must be cleaned and no food left in refrigerator. Trash placed in plastic bags and taken to garbage bin. Lights and appliances are to be turned off and the door locked upon departure.
